How To Handle Private Hospital To Government Facilities
1. Hi boss... I wanna ask you.... As kkm doctor.... Do we have the right to reject the transfer of patient from private centre?
Reply:
U can't outright reject a private referral bcos of the life of patients matter.
Of course u can give excuse no bed in ward, most importantly no ICU bed (usually if patient ventilated need to liase with anaes) or current times refer to subspecialty as we are not good at handling complicated subspecialise cases.
2.Recently... I have a case transfer from kpj.. post Apr complicated with perforated viscus.... But financial issue... So ask to transfer governement hospital... Patient quite bad sepsis require bipap ventilation..... My anasth team blame me for accept the case.... Boss... Any regulation for kkm surgeon to deal with private cases? Should ask all private complication case refer to tertiary centre?

The above answers it.
Of course, most of the time, the anaes won't be happy u take over a patient who is ventilated.
As u know, even our patients they don't ventilate n transfer out early at times.
That balance of handling anaesthetists ,we as a surgeon must know.
3.May I know how u deal with this complication from private that request transfer kkm hospital.... How u reject it?
Reply:
I try not to reject any patients referred to me bcos,
I understand the social and financial burden they would have in private as days go by.
Besides, I understand that as private surgeons, they reduce our burden of work.
I agree that, at times, they might have overdone than required.
But to me, patients come first.
I don't like to burden and stress them further.
As such, I will take over most of the patients and explain to them the grave condition they're.
The only situation I will not take over the patients with a heavy heart is when they require ICU and there is no ICU bed.ππΌ♂️
